Shoulder Pain With Overhead Press: Why It Pinches and How to Keep Pressing

Bench feels fine. Rows feel fine. Then you press overhead and something pinches near the top. Here's what's usually going on with shoulder pain during overhead press, and how we keep you pressing while we fix it.

Why does the front of my shoulder pinch at the top of a press?

The top of an overhead press is the most crowded position your shoulder gets into. The arm is fully raised, the shoulder blade has to rotate up and out of the way, and the upper back has to extend a little so the arm can finish straight over the ear.

When any of those pieces is late, the space at the front of the shoulder gets tight for a moment. The tendons of the rotator cuff and the biceps run right through that space. Pinch them a few hundred times a week and they get irritable.

The good news is that a pinch at the top is usually a movement problem before it's a tissue problem. Movement problems are trainable.

Is it the rotator cuff, the AC joint, or my thoracic spine?

Those are the three I'm sorting between in the room, and they present a little differently.

Rotator cuff irritation tends to sit on the outside or front of the shoulder, shows up in a specific arc of the press, and often bugs you reaching behind your back or lying on that side at night. The AC joint is the small bump at the top of the shoulder where the collarbone meets. If pressing on that spot reproduces your pain, or crossing the arm across your chest does, the AC joint is in the conversation.

The thoracic spine is the one people forget. If your upper back can't extend and rotate, your shoulder has to make up the difference at the top of every press. The pain is in the shoulder. The reason is often a few inches lower.

This is why I don't treat the shoulder in isolation. If the upper back and the shoulder blade aren't doing their jobs, the cuff keeps getting pinched no matter how much we calm it down.

Landmine press, dumbbells, or barbell? What to swap to and why

You almost never have to stop pressing. You have to press in a way your shoulder tolerates this month.

A landmine press is my usual first swap. The bar travels up and forward, so the arm finishes slightly in front of the head instead of directly over it. That takes the most crowded position out of the lift while keeping the shoulder blade working. Most people can press a landmine pain-free the same day a barbell hurts.

Dumbbells are the middle step. They let you turn the palms in and find a path that feels clean, and they ask each side to control itself, which usually exposes the side that's been cheating. A barbell comes back last, because it locks your hands into one path and the shoulder has to have the range to meet it.

The order isn't a punishment. It's a ladder. We climb it as your range and control come back.

How do I keep overhead volume without flaring it?

Volume is usually the lever, not the exercise. The lifters I see who flare their shoulder are rarely doing something wrong. They're doing a lot of it, and adding to it every week.

For a few weeks, keep the pressing you can do without pain climbing during the set, and cut the sets that hurt. Trade some of that volume for rows and pulling work, which most overhead athletes are short on anyway. Rowing strengthens the muscles that pull the shoulder blade back and down, and that's a big part of making room at the top of a press.

Warm-ups matter more here than in most lifts. A few minutes on upper back extension and rotation, some light shoulder blade work, and a couple of easy sets of the press itself before the working weight. It's boring. It works.

What does an assessment look for before saying "press again"?

When someone comes to see me about this, I want to know four things before I clear them to press heavy.

Can the upper back extend and rotate enough for the arm to get overhead without the lower back arching to fake it? Can the shoulder blade rotate upward smoothly, or does it hitch and shrug? Is the rotator cuff strong enough to hold the ball of the shoulder centered under load? And is there a difference side to side, because there almost always is.

From there we build a plan that trains the missing piece, keeps you pressing in the version you tolerate, and walks you back up to the barbell. The goal isn't a shoulder that survives pressing. It's a shoulder you can futureproof, so overhead work becomes something you can do for decades without thinking about it.
